Where a person is absent from work for 5 days or more the doctor can provide a Social Welfare certificate. To complete this certificate we will need your PPS number.
Certs are processed electronically.

Letters and forms requested by patients are charged for depending on their content.
Our reception staff will be able to advise on the cost in advance if applicable.
Teaching Practice
Our practice is a teaching practice for the Royal College of Surgeons in Ireland and we are fortunate to have student doctors in training attached to our practice for short periods.
As part of their training they sometimes 'sit in' on consultations but if you do not wish to have a student doctor present during your consultation please speak to reception prior to seeing the doctor.
